Proof of Identity

What is a Proof of Identity form?

If you are applying for your Criminal Conviction History from the Ministry of Justice – either for yourself or for an employer – you will need to provide one of the following forms of ID:

  • New Zealand driver licence (current or expired within the last 2 years),
  • New Zealand passport (must be signed and can be current or expired within the last 2 years),
  • Overseas passport (must be signed and current).

If you don’t have any of these, you’ll need to ask someone to confirm your identity. They do this by filling in a Proof of Identity form.
