Proof of Identity
What is a Proof of Identity form?
If you are applying for your Criminal Conviction History from the Ministry of Justice - either for yourself or for an employer - you will need to provide one of the following forms of ID:
New Zealand driver licence (current or expired within the last 2 years),
New Zealand passport (must be signed and can be current or expired within the last 2 years),
Overseas passport (must be signed and current).
If you don't have any of these, you'll need to ask someone to confirm your identity. They do this by filling in a Proof of Identity form.
The person who identifies you must:
✓ Have known you for more than 12 months
✓ Be aged 18 years or over
✓ Have a day time phone number and be contactable during normal business hours
✗ Not be a relative (a relative is a person connected by blood or marriage), and
✗ Not live at the same address.
